This form generates a report listing certified laboratories that match the groups that are selected. For a laboratory to be qualified, it must match ANY of the selected categories. For example, selecting SDWA-IOC and SDWA-VOC would produce a listing of laboratories that are certified for either SDWA IOC or SDWA VOC.

Furthermore, this report will list only the selected groups for each laboratory. Hence a laboratory may be certified for additional programs/groups outside the selected criteria which will not be included on the report.

In order to print this report, you will need to change the setting on your printer from portrait to landscape orientation.
